Do you need financial assistance to help pay for services?
You may be eligible for our sliding fee scale program or other reduced fee or no-cost services. Please complete the Sliding Fee forms or contact us to talk about your options.
A sliding fee scale adjusts the cost of counseling services according to a client’s income level household size or financial situation. Instead of charging a flat rate for everyone the fee is reduced for those who have lower income or limited financial resources.
In addition to the intake form every Sliding Fee Scale Client needs to fill out the “Open Door” Application. This form collects necessary financial information to determine eligibility. Please download and print the application to be turned in with the Intake Packet.

Qué es una escala móvil de tarifas?
Una escala móvil de tarifas ajusta el costo de los servicios de asesoría según el nivel de ingresos el tamaño del hogar o la situación financiera del cliente. En lugar de cobrar una tarifa fija para todos la tarifa se reduce para quienes tienen ingresos más bajos o recursos financieros limitados.
Además del formulario de admisión todos los clientes de la Escala de Honorarios Móviles deben completar la Solicitud de “Puertas Abiertas”. Este formulario recopila la información financiera necesaria para determinar su elegibilidad. Descargue e imprima la solicitud para entregarla junto con el Paquete de Admisión.
